Here are a few important tips on filling out the contract
1) Put instrument type: Example: Flute, clarinet, alto sax, trumpet, trombone, percussion kit.
2) Put new or used
3) In the credit/debit card section, please fill out completely if you would like to have your monthly payments automatically taken from your debit or credit card each month. Below that section in the section called “AutoPay Program Authorization” please make sure you put the billing zip code of the credit card and initial in the boxes.
4) If you do not want to do auto pay do not fill the credit/debit card section out and you will be billed through the mail.
5) The social security number and driver license number must be filled in or the contract will not be processed.
6) Please sign and date at the bottom where it says Renter: X_______________
